My experience-What to put on my doctor's note?

I am currently working with my employer to be able to work from home full-time due to migraines and vertigo (this is reasonable for the type of work I do). However, HR is requiring a specific reason I can't come into the office which runs the gamut from lights to sounds to smells to wanting to be able to ice my head in the privacy of my own home. Does anyone have advice on a specific yet vague reason I need to be WHF instead of in office? Or do I just have to suck it up and spell it out?
