My experience-Should I start with my primary care or go to a neurologist?

I have had tension headaches for years, got a bite guard and have used it pretty religiously for 4-5 years now. I have a family history of chronic migraine, though I have not been diagnosed with it myself. I'm in the middle of my worst long episode of headache/migraine yet. It's been 6 days, and the pain will intensify as the day progresses. Constant dull pressure around my forehead, throbbing pains at times from light, smells, and movement, worsening with stress (I'm a college student heading into finals week - so there's lots of stress.) I have not had pain so severe that I cry or feel nauseous, but it's impossible to focus and work consistently like this. I was sitting with a blanket over my head last night just so I could work in a bright room on my computer. It's intensifying again, I can feel it, and I'm really scared I'm developing migraines. My jaw doesn't hurt until the headache intensifies, because I then get stressed and clench my jaw, which worsens the pain. My neck hurts more as the headache intensifies. My orbitals hurt when the headache is at its worst.

Tylenol isn't touching it, and I don't want to take any pain meds that can act as blood thinners after I had a terrible nosebleed yesterday (I never get nosebleeds and I was gushing blood). It stopped and did not recur, otherwise I would have gone to the ER.

My friend who also has migraines has suggested I seek out a neurologist, which I could do so without a referral on my insurance. I'm a state away from my primary care physician and scared, but also invalidating myself. I'm probably overreacting and it's a bad tension headache. I should suck it up like I did in high school. What should I do?
