My experience-Has anyone dealt with a NY State external appeal for Botox?



My doctor has been trying to get me on Botox for a while, and my insurance keeps denying it. Finally, in August, they approved me for a treatment, but when it got time for my second treatment in November, they denied it again.

My doctor's office has escalated it to an external appeal through the state (New York), and on Thursday, I got an email saying I had 3 business days to send in any additional documents. I don't know if Thursday counts as one of those business days since I received the email near end of business day, but I'm trying to get together what I can.

Right now, I'm planning to send in my migraine tracker that dates back to Jan 2018, and see if my former doctor can provide documentation of negative side effects on Topamax, since having not tried Topamax is one of the reasons the insurance has cited for denying me. Last time I tried Topamax I couldn't eat, had the shakes, and just felt horrible on it and couldn't stay on it for more than a few days.

Any advice for other documents that might help me?

I also don't get why they approved 1 treatment but not the second. I'm on health insurance through my university (graduate student), so they've been pretty shitty anyways. I've also had issues with my Emgality getting approved.
